WHAT TO EXPECT BEFORE PLACEMENT

  1. INITIAL MEETING

Your involvement with Pathway Partners begins with a telephone call or in an initial meeting. We ask you to describe your current situation and history and we explain our role and how we can help you. We will continue to talk and/or meet in order to assure our roles and responsibilities are clear and everyone is prepared to move forward. Once you begin working with us, you will be asked to sign an agreement that delineates the terms of our proposed services, our fees and payment arrangements. You will also need to complete a Client History Questionnaire and an Authorization to Release Confidential Information. Once our contract and release forms are signed and fees are received, we will begin working with you to develop a specific plan for your family member.

  1. INFORMATION GATHERING

As you complete the Client History Questionnaire, we gather information from others, therapists, teachers, mentors, etc., which may be helpful in building a profile of your child. We will contact only those individuals for whom you have signed a release form. We will also ask you to share past testing, current school reports, individual educational plans and transcripts. We use this information to create a comprehensive Client Summary, which we send to possible programs for review.  The admissions’ staff at each place will carefully review your family member’s history and assess their potential for success at the program.

  1. PSYCHOEDUCATIONAL TESTING

Up-to-date psychological and educational tests are essential to the success of any placement. We review all previous testing; if necessary we will request new testing. For more about psychoeducational testing please visit this page.

  1. TIMING

Depending on  client and family needs, placement can occur within 24 to 48 hours of deciding in urgent situations or it may take months to determine the best course of action. In any case, we collaborate with families to develop lists of placement possibilities and we   guide you through the selection.
